FUNCTIONAL Exercise Materials

After Action Report
The After Action Report is a tool for use in evaluating the Statewide Medical and Health Exercise and developing improvement plans. Please note that the blue bar on the cover of this document indicates that this After Action Report is tailored for use in the Functional Exercise.
Controller and Evaluator Briefing
The Controller/Evaluator (C/E) Briefing slide deck can be used to provide an exercise overview to Controllers and Evaluators. It includes information on exercise logistics; evaluation guidelines; what different participants are expected to do before, during, and after the exercise; and C/E tips to complete their roles and responsibilities.
Controller/Evaluator Handbook
The Controller/Evaluator (C/E) Handbook provides an overview of the roles and responsibilities of C/Es before, during, and after the Functional Exercise. This includes exercise logistics, evaluation guidelines, as well as appendix templates. Please note that the blue bar on the cover of this document indicates that this C/E Handbook is tailored for use in the Functional Exercise.
Debriefing Slide Deck
The Debriefing Slide Deck should be utilized immediately following your organization’s exercises. It serves to capture the experience and feedback of Evaluators and Observers, as opposed to the “hot wash”, which is usually reserved for Players.
Exercise Evaluation Guides
The Exercise Evaluation Guides provide a consistent tool to direct exercise observation and data collection. Each EEG is organized by core capability, then applicable objective.
Exercise Plan
The Exercise Plan is a comprehensive document that details all aspects of the exercise for exercise planners and participants, including logistics, participant information and guidance, communications plan, scenario briefing, and more. It gives participating organizations the information necessary to take part in an operations-based Functional Exercise.
Incident Response Guide - Evacuation, Shelter in Place, Hospital Abandonment
This Incident Response Guide - Evacuation, Shelter in Place, Hospital Abandonment is intended to address issues associated with ensuring a safe environment for staff, patients, visitors, and the facility when the number of patients severely challenges or exceeds the capability and capacity of the hospital. This resource was developed by EMSA and CHA for hospitals however it can be tailored for use by clinics, long-term care facilities and other healthcare facilities. Master Scenario Events List
The Master Scenario Events List or "MSEL" provides a set of sample injects for all types of agencies and organizations to tailor and utilize for a terrorist / active shooter / explosive incident scenario.

For those who may not want to utilize the sample injects provided, this is a blank version of the Master Scenario Events List for exercise planners wishing to start from scratch.
Observer Handout
The purpose of this document is to provide an overall outline of the exercise and its objectives to Observers. It includes a schedule of exercise events, scenario briefing, and more.
Participant Feedback Form
The Participant Feedback Form has been provided for exercise planners to distribute to all participants at the conclusion of the exercise. Results from the Participant Feedback Form should be documented in the After Action Report. As such, the After Action Report includes language for easy insertion of these results.
Player Badge
The Player Badge can be provided to Players as a source of identification. The Player Badge contains quick reference information about the exercise including the exercise schedule, safety information, and more. The Player Badge is designed to be cut to a smaller but legible size for ease of reference by Players.
Player Handout
The purpose of this document is to provide an overall outline of the exercise and its objectives to Players. It includes a schedule of exercise events, scenario briefing, and more. The Player Handout may be provided to Players instead of, or in addition to, the Exercise Plan.
Player Briefing Slide Deck
The Player Briefing Slide Deck can be used to provide an exercise overview to players. It includes information on exercise assumptions and artificialities; exercise schedule; player safety; player communications; and what players are expected to do before, during, and after the exercise.
Press Release
The Press Release template is a written communication reporting specific, but brief information on the exercise that can be used to advise the media of your participation in the Statewide Medical and Health Exercise.
Sample Timeline for Exercise Planners
The Sample Timeline for Exercise Planners is a helpful tool for exercise planners keeping track of meetings, deadlines, and activities for exercise prep. This timeline features key outcomes for each planning meeting.
Save the Date
The Save the Date template is a document that can be used to alert potential participating individuals and agencies of important dates, times, and meetings for the exercise.
Waiver Forms
The purpose of this form is for participants to release the California Department of Public Health, California Emergency Services Authority, and your jurisdiction/organization/facility from liability as well as grant permission to be photographed.
Actor Card Instructions
The Actor Cards Excel Table features sample profiles for both patients and family members. The Actor Card Excel Table can be modified and merged to a Word document using the Mail Merge function to create customized Actor Cards.

For those who want to use the sample actor profiles provided in the excel sheet exactly as they are, this document offers a completely mail-merged set of ready-to-use cards that can be printed and used immediately. Actor Cards contain the signs and symptoms the actors will portray, as well as information for other Players (medical providers, first responders, etc.).
