Tabletop Exercise Materials

After Action Report
The After Action Report is a tool for use in evaluating the Statewide Medical and Health Exercise and developing improvement plans. Please note that the green bar on the cover of this document indicates that this After Action Report is tailored for use in the Tabletop Exercise.
Certificate of Participation
The Certificate of Participation is offered to those individuals who have participated in the Statewide Medical and Health Exercise in order to recognize their contribution to emergency preparedness.
Change the Scenario
This document explains to exercise planners how the Tabletop Exercise can be altered to accommodate an alternate scenario.
Controller/Evaluator Handbook
While a Controller/Evaluator Handbook is not typically used for a Tabletop Exercise, it can be used for complex Tabletops. This document provides an overview of the roles and responsibilities of Controllers and Evaluators before, during, and after the Tabletop Exercise. This includes exercise guidelines, rules, and evaluation guidelines, as well the Tabletop Exercise scenario and discussion questions. Please note that the green bar on the cover of this document indicates that this Handbook is tailored for use in the Tabletop Exercise.
Exercise Evaluation Guides
The Exercise Evaluation Guides provide a consistent tool to direct exercise observation and data collection for the Tabletop Exercise.
Facilitator Guide
The Facilitator Guide is intended to prepare and assist facilitators and staff in conducting the Tabletop Exercise. It contains general information on the overall exercise process, necessary materials, and discussion questions.
Incident Planning Guide: Active Shooter
This Incident Planning Guide - Active Shooter is intended to address an incident involving an active shooter, defined as an individual actively engaged in killing or attempting to kill people in a confined and populated area. This resource was developed by EMSA and CHA for hospitals however it can be tailored for use by clinics, long-term care facilities and other healthcare facilities. Observer Handout
The purpose of this document is to provide an overall outline of the exercise and its objectives to Observers. It includes a schedule of exercise events, scenario briefing, and more.
Participant Feedback Form
The Participant Feedback Form has been provided for exercise planners to distribute to all participants at the conclusion of the exercise. Results from the Participant Feedback Form should be documented in the After Action Report. As such, the After Action Report includes language for easy insertion of these results.
Player Handout
The purpose of this document is to provide an overall outline of the exercise and its objectives to Players. It includes a schedule of exercise events, scenario briefing, and more. The Player Handout may be provided to Players instead of, or in addition to, the Situation Manual.
Situation Manual
The Situation Manual (SitMan) is the primary support document for the Tabletop Exercise. This document may be used by all participants during the course of the exercise and includes roles, instructions, scenario modules, discussion questions for different participating agencies, and informational appendices.
Tabletop Slide Deck
The Tabletop Slide Deck is used to facilitate the Tabletop discussion, outlining the modules and discussion questions in the same order as the Situation Manual. Tabletop Exercise planners are encouraged to tailor these slides according to the objectives and capabilities targeted by their jurisdiction/organization/agency.

Exercise planners are welcome to use this formatted report template for creating supplementary reports, tools, appendices, or any other documents you can think of.
